A supervisory body must keep a written record of the following information—

(a)

each request for a review that is made to them;

(b)

the outcome of each request;

(c)

each review which they carry out;

(d)

the outcome of each review which they carry out;

(e)

any variation of an authorisation made in consequence of a review.
