Applications under paragraph 5(1) of Schedule 13f1 must be made in writing to the Commissioners and include such information and particulars as they may prescribe.
A notification under paragraph 11 of Schedule 13f2 must be made in writing to the Commissioners and include such information and particulars as they may prescribe.
The notification required by paragraph (2) must be given to the Commissioners before the end of a period of 30 days beginning with the day after the day that—
any member becomes aware that it ceases to be eligible under paragraph 11(1) of Schedule 13; or
the representative member ceases to have an established place of business in the United Kingdom.
