Whether the pupil has special educational needs and, if so, confirmation of—

(a)

the pupil’s primary need and any secondary need identified;

(b)

the SEN provision type forming part of the graduated approach adopted pursuant to “The Special Educational Needs Code of Practice for Wales”^f1, which was issued under section 313 of the 1996 Act, that is being made for that pupil; and

(c)

the support being provided.
