Regulation 3 Information to be contained in all notifications under section 39(1) of the Act
A notification under section 39(1) of the Act must contain—
(a)
the name of the service provider;
(b)
the name and type of regulated service to which the notification relates;
(c)
the place or places to which the notification relates;
(d)
the date on which the notification is sent; and
(e)
the paragraph of section 39(1) of the Act or of regulation 10 under which the notification is sent.
