In respect of applicants other than local authorities and Local Health Boards—

(a)

a business plan;

(b)

a reference from a bank expressing an opinion as to the applicant’s financial standing;

(c)

details as to projected cash flow in respect of the individual or organisation applying to be registered as a service provider.
