the words “medicated premixture”;

the proprietary name of the veterinary medicinal product and the authorisation number;

the strength;

the inclusion rate of the premixture into the feedingstuff;

the level of the active ingredient in the final feed;

warnings and contra-indications;

withdrawal period;

the expiry date;

any special storage instructions;

where a prescription is required, a statement to this effect;
