Regulation 4 Prescribed forms (gambling)

The forms set out in Schedules 3 to 13 to these Regulations are prescribed as the Welsh or bilingual versions of the forms set out in Schedules 1 to 11 to the Gambling Act 2005 (Premises Licences and Provisional Statements) Regulations 2007f1 (“the 2007 Regulations”) as indicated in the table in Schedule 27 to these Regulations.

(a)
column 1 lists the Welsh or bilingual versions of the forms as set out in these Regulations by reference to the corresponding forms set out in the 2007 Regulations listed in column 2, and
(b)
column 2 lists the forms as these are set out in the 2007 Regulations.
