Article 10 Amendments to Schedule 5 – Forms

There shall be inserted in Schedule 5 to the Rules, following Form 4.30 (Scot)^f1, the form set out in Part 1 of the Schedule to this Order.

For Page 2 of Form 4.4 (Scot) in Schedule 5 to the Rules substitute the page set out in Part 2 of the Schedule to this Order.
