(1)
A permit shall cease to have effect upon being surrendered by notice given by the holder of the permit to the issuing licensing authority.
(2)
A notice under sub-paragraph (1) must be accompanied by—
(a)
the permit, or
(b)
a statement explaining why it is not reasonably practicable to produce the permit.
